Application is not invoked after call scheduled

Team,
After scheduled calls, some return status 'completed', some return 'failed'.
In both 'completed' and 'failed', there exists such a situation that some related call log are missed for related scheduled call.
And the related postcall-webservice is not executed also.
Could you please help us to check what kind of scenario can cause this?
