How can each of our customers using IVR have their own local phone number to display on their patient's caller ID, right now they all show 866.232.7188?

Hi,

You can use the tel url format to override the default caller ID setup for your outbound account.

For example:

tel:+19991234567;ani=2078562433

However, please keep in mind that there is no 100% guarantee that the ANI you set will be presented to the callee.
